Choose the right partners to invest in and work with

In line with its focus on outcomes, Daly said the Medtronic Foundation is aiming to build closer, more collaborative partnerships with a streamlined set of partners to keep goals tightly aligned over time. Instead of high-volume but small-dollar grants, the Medtronic Foundation uses a data-driven selection process to invest deeper in a smaller group of partners in a more strategic way.

Its strategy also involves incremental financing based on milestones that enable changing methods based on what does and does not work. “We can say we want to pivot here and we’re going to actually change course and shift the agreement this way,” Daly said.

It allows us to act more nimbly and be more responsive.

For example, the Medtronic Foundation is supporting microfinance organization Grameen America, which leads a CHW and health promotion program run in a nonclinical setting to reach low-income women. In South Africa, it is supporting mothers2mothers to help women and families managing chronic conditions. In Bangladesh, the Medtronic Foundation is helping BRAC develop new patient-centered care models and increase its CHW staff to support those with chronic conditions.

With the Puerto Rico Science, Technology & Research Trust, the Medtronic Foundation focuses on disaster preparedness, resilience, and strengthening the capacity of “promotoras de salud” to provide home- and center-based care for chronic care management. And in Mysuru, India, it is working with Karnataka Health Promotion Trust to expand 21 primary care facilities and improve outcomes for those with diabetes and high blood pressure.
